AI has a couple of important roles to play.
One is to help organizations identify these patients as they’re moving through the system. Identify them both in the community or when they show up in the acute care setting, enroll them in care programs that are tailored for their needs, and potentially helping those patients transition back into the community with the appropriate care and support they need in the home setting to avoid further needs and requirements for acute care services.
To do that, not only you need stratification to identify these patients, but granular stratification to identify their specific risks and needs that the care team needs to focus on so.
