Think of it like this: “This patient has a 78% predicted risk for post-discharge ED return. Has anyone confirmed follow-up?”

Sample Use Case:
A patient flagged for post-discharge sepsis risk triggered early engagement with Infectious Disease consult and arrangements with home health agency. This minimizes the likelihood of complications associated with infections and reduces the chance of readmission and helps the patient stay out of the hospital after discharge.
